Historically, within a smaller community, citizens could achieve renown by becoming the best in their field, locally. The citizens a few towns over might never hear of you, but who cares about them? Everyone has his place within smaller societies.
I don’t mean to say localism/decentralisation is a cure-all, but it’s another facet.
Yes, very frequently a town might have someone who is the “best singer” or “best actor” without them having to leave or adopt unhealthy lifestyles.
